Planned Absence Request Form

If you are planning on having your student miss one or more days of school, please fill out this form.

Minneapolis Attendance Policy 5100 allows up to 5 days per year for excused family activity—more than 5 days are unexcused. Please inform us of your planned family activities. If you have any questions, please call the Health Office at 612-668-4007.
